Three stages feed the loss. Stage 1: enquiries lost before they reach the Doctor (missed calls, slow callbacks). Stage 2: OPDs that did not convert to a procedure recommendation. Stage 3: recommended procedures that never happened, partly recovered by follow-up. The leakage at each stage is added together and multiplied by your average procedure value.
